Title: Beyond Adhesiolysis: Elevating Adhesion Prevention in Hysteroscopic Surgery

Intrauterine adhesions (IUAs) remain an important and often underrecognized consequence of procedural trauma to the endometrial basalis, with potential implications for fertility and future pregnancy outcomes. This session will explore what current evidence tells us about IUA incidence, pathogenesis, reformation following adhesiolysis, and downstream reproductive and obstetric consequences. Faculty will examine strategies for optimizing surgical outcomes, including the mechanistic and design characteristics of intrauterine systems that might facilitate adhesion-free repair. These elements are embodied in the Juveena Hydrogel System: liquid transcervical delivery, retrograde cavity filling, rapid in situ polymerization, conformance to the contours of the uterine cavity, and continuous separation of apposing surfaces of the endometrial cavity during the early postoperative repair interval. The efficacy of the system has been demonstrated in a recently published multicenter, subject- and evaluator-blinded pivotal randomized controlled trial with blinded second-look hysteroscopy. The session will then translate these principles into clinical practice by reviewing the Juveena Hydrogel System procedural workflow.
